The report indicated that inconsistencies between the execution layer and the proving layer states, under specific combinations of cross-function calls and rollbacks, led to errors in the execution layer's recording of rolled-back state writes. This caused transaction execution anomalies, with related transactions failing to achieve L1 finality confirmation. The incident triggered block reorganization, resulting in approximately 18 minutes of on-chain activity being rolled back. This marks the second major disruption since 2025; last September, a sequencer vulnerability caused an outage of over 5 hours and rolled back approximately 1 hour of on-chain activity.
